Please describe the specific items that you need * When possible, include measurements, descriptions, and specifications (e.g. gas vs electric, flooring square footage). Your request does not guarantee that we will be able to meet item specifics or fulfill the request. Examples include but are not limited to: refrigerators, window AC units, doors, windows, dishwasher, washer, dryer, gas stove and oven, furniture, sink, toilet, etc.

Why do you need this item/items? *

Does your household receive any type of public assistance? * SNAP, EBT, Section 8 housing, Disability, Energy Assistance Program, etc. Not sure? Check the federal poverty guidelines to determine your eligibility.

Can you provide verification of need in some form? * Tax return, SNAP card, caseworker verification, police report, letter from a community leader, etc. You'll need to present this during your appointment. If you cannot provide documentation, your application will still be considered.
